The paper reveals how the public health organizations of Buenos Aires Province work, focusing on changes brought about by private initiatives inside the institutions. Non-regulated short, middle, and long-term actions from healthcare professionals and workers, in combination with the institutional structure of power, have effects on the management and on the results. These unofficial initiatives can contribute to the perpetuation of the clientelist politics or to the bottom-up change from within. The informal dimension of healthcare organizations is constitutive of their daily practices, giving another meaning to public policy expressed in formal terms.
