The article addresses the way in which a profile of a hospitalized patient, defined as social, is constructed as a medicalprofessional problem by staff, concurrent and resident doctors, within the framework of a Medical Clinic service that works in an Acute Hospital, Autonomous City of Buenos Aires. This patient profile is of interest since, beyond the resolution of the acute episode that determined his admission, he must remain in the hospital due to certain conditions that avoid adequate care and accommodation. The resulting ambiguous situation (which, strictly speaking, places it outside the jurisdiction of the Medical Clinic), generates a series of tensions, contradictions and dilemmas that professionals expose when referring to the dynamics of classification and management of hospital routines associated with the social patient. The paper exposes three tensions in particular: (a) those that emerge between medical colleagues; (b) those associated with the “clash” between medical order and the organizational context (hospital order); and (c) those related to the relationship between the medical order and the patient's lifeworld. The paper is based on an interpretativequalitative research approach of a master's thesis, through indepth interviews, aiming to reconstruct the meaning structures involved.
