This article examines the institutional fabric of healthillness, care, and treatment processes in Argentina, within the context of neoliberal governmentality during the postpandemic period. The objective is to analyze the interventions of social workers in public, private, and social security institutions in the southern zone of the Buenos Aires Metropolitan Area (AMBA, Argentina), between 2021 and 2024. This was a qualitative study based on 10 semistructured interviews with primary care professionals, hospital staff, and social security providers, complemented by participant observations in these settings (2024), alongside thematic and contextual analysis. The results focus on identifying three processes: 1) job losses and state retrenchment; 2) the increasing complexity of therapeutic pathways; and 3) institutional decline (gaps between regulations and practices, and postpandemic reconfigurations). The discussions highlight the tensions between the rhetoric of rights and material precariousness, with social work professionals mediating to rebuild accessibility. The conclusion establishes that accessibility emerges as a contested relationship, requiring robust institutions to realize the right to health.
