he obstetric antiphospholipid syndrome is an heterogeneous disorder that can cause severe morbidity like late fetal losses, intrauterine growth restriction, placental insufficiency, early preeclampsia, abruptio placentae, HELLP syndrome or recurrent abortions without other explanation. Antiphospholipid antibodies can induce activation of the endothelium, monocytes and platelets, increase the expression of tissue factor and thromboxane A2 and activate complement. These antibodies can decrease placental hormone production and trophoblast intercellular fusion and invasion and provoke a trophoblast “dysfunction”. Among the antiphospholipid antibodies the “lupus anticoagulant” seems to be the strongest predictor of thrombotic and obstetric complications. The standard treatment is based on the combination of heparin and low dose aspirin. Heparin is beneficial through its capacity of inhibiting complement activation. The benefit on the newborn rates shown by the early studies comparing unfractionated heparin and aspirin versus aspirin has not been confirmed by further studies with low molecular weight heparin. The choice of the treatment should be individualized as the available studies have limited scientific value
