Trichomoniasis urogenital: actualización en clínica, diagnóstico, morbilidad y tratamiento

Abstract

La Trichomoniasis urogenital permanece como una enfermedad extendida a través del mundo y relegada en su atención. Más del 50% de las personas infectadas son asintomáticas, desestimándose probablemente por esta causa, el diagnóstico y el tratamiento. En mujeres gestantes se ha comprobado significativa asociación de T.vaginalis con partos prematuros, ruptura temprana de bolsas y recién nacidos con bajo peso. Facilita la infección por HIV y HPV. Las investigaciones sobre una posible relación con cáncer cérvico uterino y de próstata tiene fuerte evidencia en algunos estudios mientras que otros muestran resultados discordantes. En nuestro país se encuentra olvidada en su atención al ser los programas de vigilancia y control muy limitados. Se requieren mayores investigaciones sobre epidemiología, clínica, diagnóstico y tratamiento.

Urogenital Trichomoniasis remains as an extended disease throughout the world and relegated with regard to its attention and treatment. More than 50% of the infected persons are non-symptomatic, probably relegating diagnosis and treatment because of this reason. In gestating women, it has been proved a significant association of the presence of T.vaginalis with and increased risk of preterm birth, preterm premature rupture of membranes and low-weight new-borns. Additionally, T.vaginalis infection increase sexual transmission of HIV and HPV. Research concerning possible relationships with uterine, cervical an prostatic cancer have gathered substantial evidence in some studies, while others show discordant results. Clinical diagnosis is not precise because of the high percentage of non-symptomatic cases, whilst cultures of Trichomonas constitute a highly valuable tool where PCR implantation is not possible because of its high costs. In our country, the attention and treatment of the disease is relegated, with very limited monitoring and control programs. More research is needed in the fields of epidemiology, clinic, diagnosis and treatment of the disease.
