Consideraciones éticas en torno al uso del diagnóstico en salud mental: aportes de la “pequeña ética” de P. Ricoeur

Abstract

La clínica en Salud Mental toma como herramienta fundamental al diagnóstico diferencial. Es lo que permite el abordaje a cualquier tratamiento posible. Esta característica instrumental no está desprovista de desafíos éticos que merecen ser reflexionados. El riesgo de la mala aplicación de sus categorías o de su abuso, conlleva un daño que excede al simple “error” de pronóstico y tratamiento de la persona que consulta. Así, el propósito de este trabajo radica en la reflexión sobre los dilemas éticos que el mal uso del diagnóstico acarrea en los usuarios de salud mental en la actualidad, a partir de los aportes de Ricoeur sobre la intencionalidad ética. La razón es poder extender el horizonte del diagnóstico como mera herramienta para complejizarla hacia una instancia dialógica que, por la referencia al otro, exige a su vez, responsabilidad en las consecuencias mismas que implica su uso.

The clinic in Mental Health takes as fundamental tool the differential diagnosis. It is what allows the approach to any possible treatment. This instrumental characteristic is not devoid of ethical challenges that merit reflection. The risk of misapplication of their categories or of their abuse entails damage that goes beyond the simple “error” of prognosis and treatment of the person consulting. Thus, the purpose of this work lies in the reflection on the ethical dilemmas that the misuse of the diagnosis carries on the users of mental health at present from the contributions of Ricoeur with its definition on the ethical intentionality. The reason is to be able to extend the horizon of the diagnosis of a mere tool to complex it as a dialogical instance that, by reference to the other, demands responsibility in the same consequences that implies its use.
