Construcción de indicadores de accesibilidad al sistema de salud sensibles a la cultura

Abstract

Se presenta el análisis de publicaciones correspondientes a estudios de accesibilidad/acceso al sistema de salud que han incluido en sus indagaciones la dimensión cultural del fenómeno en estudio. Las sociedades modernas se caracterizan por una amplia diversidad cultural, como consecuencia de ello, coexisten diversas formas de comprender la salud y la enfermedad y modos distintos de respuesta. Ello ha llevado a una serie de inadecuaciones entre la prestación de servicios de salud y la población beneficiaria. Metodología: se realizó una revisión crítica de la literatura científica sobre acceso/accesibilidad cultural al sistema de salud para identificar la interpretación de la dimensión cultural de la accesibilidad y su operacionalización. Resultados: Fueron seleccionados 65 artículos de las bases de datos LILACS, Pubmed y Scielo. Se observa un aumento progresivo de artículos sobre este tema a medida que avanzan los años. La población que fue mayormente indagada presenta como caracteristica la etnicidad. Predominan los estudios cualitativos. Las formas de nominación de las variables y sus operacionalizaciones varían tanto en los estudios cualitativos como cuantitativos. Las variables representaciones son las más utilizadas. Conclusiones: se observa una pluralidad de formas de comprender y operacionalizar la dimensión cultural en los estudios de accesibilidad al sistema de salud señalando la necesidad de seguir discutiendo la construcción de indicadores culturalmente sensibles.

This article presents the analysis of publications for studies of accessibility to health system have included in their inquiries the cultural dimension of the phenomenon under study. Modern societies are characterized by a wide cultural diversity in its population, as a result, the population coexist in different ways of understanding health and disease and different ways of response. This has led to a number of inadequacies between the provision of health services and the beneficiary population. Methodology: A critical review of the scientific literature on access / accessibility to health care culture is performed to identify the interpretation of the cultural dimension of accessibility and its operationalization. Results: A total of 65 articles were selected from the Latindex, Pubmed and Scielo databases. There is a progressive increase of articles on this topic as the years progress. The population that was most investigated presents ethnicity as a characteristic. Qualitative studies predominate. The forms of nomination of variables and their operationalization vary both in qualitative and quantitative studies. The representations variables are the most used. Conclusions: we observed a plurality of ways of understanding and operationalising the cultural dimension in studies of accessibility to health care. Deeping this discussion becomes a fertile field and a necessary contribution to the study of the health situation of indigenous peoples and their relationship with the health system.
