La justicia territorial y la accesibilidad a la salud en áreas metropolitanas

Abstract

Los desequilibrios territoriales, generados por los procesos de producción espacial imperantes en el contexto neoliberal latinoamericano, alejan a las regiones de la senda del desarrollo. Los servicios públicos, particularmente la salud, son un derecho que contribuye al desarrollo. Es por ello que, desde la perspectiva de la ordenación de territorio, es relevante tanto la disponibilidad de los equipamientos, como su distribución en el territorio, determinando la posibilidad de acceder al servicio y beneficiarse, mejorando su calidad de vida. El análisis de la accesibilidad geográfica de la población a los centros de salud de mayor jerarquía (emergencias, especialidades y tratamientos de complejidad) representa un instrumento metodológico, de diagnóstico y prospectiva para obtener una aproximación a la justicia territorial, en el ámbito metropolitano. A través de las herramientas de análisis espacial de los sistemas de información geográfica, se establecen áreas de accesibilidad diferencial en una metrópoli intermedia argentina, Santa Fe, en proceso de consolidación e integración. El concepto de justicia territorial se vincula a otros como justicia ambiental y espacial, con los cuales comparte el enfoque de la sustentabilidad desde el pensamiento ambiental latinoamericano; se concibe como aquella que promueve una cultura territorial vigilante de los valores e intereses puestos en juego en las dinámicas actuales, proponiendo mecanismos de redistribución y reacomodos territoriales, redefiniendo los roles del estado, el mercado y la sociedad.

The territorial imbalances are generated by the processes of spatial production typical of the neoliberal Latin American context, distancing the regions from the path of development. Health is a right that contributes to development. In the analysis of public services, both the availability of the equipment and its distribution in the territory are relevant, from the perspective of territorial planning. This situation determines the possibility for society to access and improve its quality of life. Using the tools of geographic information systems and free access and open source applications (QGIs, OpenStreetMap, OpenRouteService), through network analysis and isochron calculation, areas of differential accessibility to the 3 third level centers are obtained, in an intermediate Argentine metropolis, Santa Fe, in the process of consolidation and integration. The analysis of the geographical accessibility of the population to the highest health centers (emergencies, specialties and complex treatments) and the resulting cartographic product, represent a diagnostic and prospective instrument to obtain an approach to territorial justice in the metropolitan area. The concept of territorial justice is linked to others such as environmental and spatial justice, with which it shares the approach to sustainability from Latin American environmental thought; It is conceived as one that promotes a territorial culture that monitors the values and interests at stake in current dynamics, proposing mechanisms of redistribution and territorial rearrangements, redefining the roles of the state, the market and society.
