Desigualdades de género en la accesibilidad a la salud integral en juventudes trans y no binarias de Córdoba, Argentina

Abstract

La presente investigación buscó comprender las vivencias de jóvenes trans y no binaries de la provincia de Córdoba (Argentina, 2020/2021), en relación con la accesibilidad a la salud integral, precisando aspectos facilitadores y obstaculizadores en prácticas de salud desde su propia perspectiva. Interesó precisar en qué medida se aplican legislaciones vinculadas a género y salud especificando en qué punto su cumplimiento o incumplimiento afecta las posibilidades y condiciones de acceso a la salud en esta población. La óptica éticoepistemológica y política que orientó el trabajo recupera la perspectiva de género, trans-feminista, y de derechos humanos. Se realizó un estudio empírico cualitativo, mediante la realización de entrevistas semiestructuradas a 17 jóvenes trans y nobinaries residentes en la provincia de Córdoba, con edades comprendidas entre los 16 y 35 años. El análisis de datos se realizó en el marco de la teoría fundamentada en los datos. Los resultados subrayan la saliencia del trato de les efectores del sistema de salud en la configuración de aspectos facilitadores y obstaculizadores del acceso a la salud. A su vez, se ratifica la deficiente formación en perspectiva de género y la falta de conocimiento de la Ley de Identidad de género de algunes efectores del sistema sanitario, así como la ausencia de redes sociales y afectivas de contención y apoyo como otras dimensiones relevantes que obstaculizan el acceso a la salud. Asimismo, se subrayan dificultades situadas en la intersección entre género y edad, con predominancia de una matriz adultocéntrica que obstaculiza el acceso a derechos. Por último, se presentan algunas particularidades vinculadas con el acceso a la salud integral en el marco de la pandemia por COVID-19.

This research sought to understand the experiences of trans and non-binary youth in the province of Córdoba (Argentina, 2020/2021), in relation to accessibility to health care services, specifying aspects that facilitate or hinder the access. Accordingly, we were interested in specifying the extent to which gender and health legislations are applied, specifying how its compliance or non-compliance affects the possibilities and conditions of access to health in this population. The ethical-epistemological and political perspective that guided the work was based on a gender, trans-feminist and human rights perspective. A qualitative empirical study was carried out through semi-structured interviews with 17 trans and nonbinary young people living in the province of Córdoba, aged between 16 and 35 years old. The data analysis was carried out within the framework of grounded theory. The results underline the importance of health system providers' treatment in the configuration of health access facilitators and hinderings. At the same time, the deficient training in gender perspective and the lack of knowledge of the Gender Identity Law of some health system providers, as well as the absence of social and affective networks of containment and support are ratified as other relevant dimensions that hinder access to health care. Likewise, difficulties located at the intersection between gender and age are highlighted, with a predominance of an adult-centered matrix that hinders access to rights. Finally, some particularities related to access to comprehensive health care in the context of the COVID-19 pandemic are presented. Key-words: trans youth, non-binary, health services, accessibility.
