Salud y ética feminista del cuidado: Desafíos en el contexto COVID-19

Abstract

La pandemia por COVID-19 ha puesto en jaque diversas dimensiones del orden social, siendo la salud una de ellas. La pandemia resituó a la salud en el centro del debate público, y nos dio la posibilidad de reinstaurar discusiones centrales para la construcción de un orden social más justo. Sabemos que en el sistema de salud conviven en conflictiva tensión visiones que comprenden a la salud desde un modelo médico hegemónico y aquéllas que la conciben como un derecho fundamental desde miradas críticas. En este contexto, se torna necesario propiciar otro modo de pensar la salud. Los feminismos, desde distintos lugares, vienen proponiendo hace tiempo la necesidad de avanzar hacia un modelo social de la salud, que considere sus múltiples determinaciones, entre las que destaca el género. En este marco, es urgente preguntarnos cómo se está interviniendo en nuestros cuerpos y cómo ello impacta en nuestras subjetividades, a la vez que indagar cómo los cuidados formales desde los servicios de salud están siendo proporcionados. Sobre este último punto, debe recordarse la feminización que caracteriza al sector salud y sus impactos evidenciados en los costes en los equipos durante la pandemia.Desde una mirada feminista, el cuidado constituye una dimensión fundamental del orden social. Si bien históricamente los cuidados se asignaron a los hogares (y en ellos, de forma casi exclusiva, a las mujeres), lo cierto es que existen diversas instituciones que intervienen en las dinámicas sociales de cuidado. En el ámbito de la salud, tanto el Estado, como las organizaciones comunitarias, como el propio mercado (a través de procesos de mercantilización) forman parte de la organización social de los cuidados. En ese contexto, las mujeres ocupan roles centrales en las tareas de cuidado, tanto en su participación en las instituciones sanitarias, como – por ejemplo- a partir de su involucramiento en comedores comunitarios en el marco de una pandemia que se ha llevado la vida de varias de ellas. Por ende, la pandemia es una ocasión histórica inigualable para abordar de manera crítica dicho ordenamiento. Hemos revalorizado que cuidar es una tarea fundamental para toda la sociedad, indispensable para el bienestar de todes, ya que sin cuidados no hay bienestar. El cuidado trasciende el género, no es tarea de mujeres exclusivamente, sino que se instituye como una forma de estar en el mundo, que exige de una responsabilidad por les otres. Desde esa mirada, el cuidado de les otres es, al mismo tiempo, cuidado de nosotres mismes, y constituye una acción reflexiva no sólo del pensamiento sino también del afecto. La pandemia vino a subrayar la importancia de la ética del cuidado y de la salud colectiva, indicando lo imprescindible de los valores de justicia, solidaridad y bien común. Una mirada desde la ética feminista del cuidado subraya la importancia de darle voz a los/as pacientes, respetar su integridad, sus emociones, sus vínculos. Ello supone que para afrontar esta pandemia es necesario no sólo dotar a los equipos de salud de condiciones materiales, sino también de herramientas emocionales, reflexivas y de una perspectiva de género. La ética del cuidado debería permear nuestras prácticas como agentes de salud, pero también las políticas públicas y sanitarias. En este simposio, proponemos analizar de manera crítica y reflexiva al sistema de salud y las políticas sanitarias, aportando desde un enfoque propositivo herramientas y debates desde el campo de los feminismos. Para ello, invitamos a pensar el rol de las políticas públicas y cómo plasmar la ética de los cuidados en la atención integral a la salud de la población en un momento histórico como el actual, al tiempo que revisar desde una mirada feminista la noción de vulnerabilidad.

The COVID-19 pandemic has jeopardized various dimensions of the social order such as health and well-being. The pandemic put health back at the center of public debate, and gave us the opportunity to re-establish central discussions for the construction of a more just social order. We know that within the health system coexist visions that understand health from a hegemonic medical model with those that conceive it as a fundamental right from critical points of view. In this context, it becomes necessary to promote another way of thinking about health. Feminisms, from different places, have been proposing for a long time the need to move towards a social model of health, which considers its multiple determinations, among which gender stands out. Within this framework, it is urgent to ask ourselves how our bodies are being intervened and how these impacts on our subjectivities, while at the same time investigating how formal care from health services is being provided. Regarding this last point, the feminization that characterizes the health sector should be remembered. From a feminist perspective, care constitutes a fundamental dimension of social order. Although historically care was assigned to homes (and in them, almost exclusively, to women), the truth is that there are various institutions that intervene in the social dynamics of care. In the field of health, both the State, as well as community organizations, and the market itself (through commercialization processes) are part of the social organization of care. In this context, women occupy central roles in care tasks, both in their participation in health institutions, and - for example - from their involvement in community kitchens in the framework of a pandemic that has taken the lives of several of them. Therefore, the pandemic is a unique historical occasion to critically address this order. We have revalued that caring is a fundamental task for all of society, essential for the well-being of all, since without care there is no well-being. Care transcends gender, it is not the task of women exclusively, but it is instituted as a way of being in the world, which requires responsibility for others. From that point of view, caring for others is, at the same time, caring for ourselves, and constitutes a reflective action not only of thought but also of affection. The pandemic underscored the importance of the ethics of care and collective health, indicating the essential of the values of justice, solidarity and the common good. A look from the feminist ethics of care underlines the importance of giving patients a voice, respecting their integrity, their emotions, their ties. This means that in order to face this pandemic it is necessary not only to equip health teams with material conditions, but also with emotional, reflective tools and a gender perspective. The ethics of care should permeate our practices as health agents, but also public and health policies. In this symposium, we propose to critically and reflectively analyse the health system and health policies, providing tools and debates from the field of feminism. To do this, we invite you to think about the role of public policies and how to translate the ethics of care in comprehensive health care for the population in a historical moment such as the current one, while reviewing the notion of vulnerability from a feminist perspective.
