Aplicaciones de la hormona de crecimiento humana recombinante en HIV: Efectos sobre el aparato inmune

Abstract

The present work carries out a bibliographic research on the clinical use of RhGH in adult HIV patients in its immunomodulatory role by means of Boolean search in Pubmed of the terms RhGH and HIV in articles of clinical trials, only in adults from 19 or more years of age, until January 2019. There were 27 articles on clinical trials, of which 8 articles were analyzed with the specific objective of investigating the effects on the immune system of RhGH in HIV cases, of which four show good evidence (A1b). The doses tested are different for each trial and range from: 0.7 to 4 mg / day, likewise the duration of it from: 12 weeks to 40 weeks. In all cases, it was concomitant with efficient antiretroviral treatment. These clinical studies show an increase in the CD4 cell count (discrete) and an evident trophic immune effect on the thymus and cell activation (improvement of the immune response). It remains to clarify in future research the dose and duration of optimal treatment, the use in C and / or B viral co-infections and the impact of RhGH on the potential reduction of some of the HIV reservoirs.

El presente trabajo realiza una investigación bibliográfica sobre el uso clínico de RhGH en pacientes adultos HIV en su rol inmunomodulador mediante búsqueda booleana en Pubmed de los términos RhGH y HIV en artículos de ensayos clínicos, únicamente en adultos a partir de los 19 o más años de edad hasta enero del 2019. Se obtuvieron 27 artículos sobre ensayos clínicos, de estos se analizaron 8 artículos que tenían como objetivo especifico investigar los efectos sobre el aparato inmune de RhGH en casos HIV, de los cuales cuatro muestran buena evidencia (A1b). Las dosis ensayadas son distintas para cada ensayo y van desde: 0.7 a 4 mg/día, de igual modo la duración del mismo desde: 12 semanas a 40 semanas. En todos los casos fue concomitante a tratamiento antirretroviral eficiente. Estos estudios clínicos demuestran un aumento del recuento celular CD4 (discreto) y un efecto trófico inmune evidenciable sobre el timo y la activación celular (mejoría de la respuesta inmune). Queda por aclarar en futuras investigaciones la dosis y duración del tratamiento optima, el uso en co-infecciones virales C y/o B y el impacto de RhGH en la potencial reducción de algunos de los reservorios de HIV.
