Importancia de la vacunación posterior al trasplante de células progenitoras hematopoyéticas: Vacunación de una Población en Riesgo

Abstract

El trasplante de células hematopoyéticas (TCH) es una terapia celular que consiste en la transferencia de un sistema inmune de un donante a un receptor mediante el reemplazo de estas células. El TCH representa la mejor opción de tratamiento curativo para enfermedades hematológicas malignas y no malignas. Materiales y métodos: se realizaron búsquedas bibliográficas en PubMed del Instituto Nacional de Salud de Estados Unidos de Norteamérica (National Institute of Health, NIH) y se consultó al Ministerio de Salud de la República Argentina. El TCH está asociado con un riesgo incrementado de infecciones, de las cuales algunas son prevenibles por vacunación. Las guías recomiendan dosis repetidas de vacunas principalmente inactivadas para lograr respuestas inmunes de larga duración. Objetivo: la eficacia de la inmunización es a menudo obstaculizada por enfermedad de injerto contra huésped o enfermedades oportunistas severas. Conclusión: las recomendaciones de vacunas para receptores de TCH alogénicas y la discusión de los desafíos y las direcciones futuras que conllevan a la inmunización de estos pacientes resultan de vital importancia.

Hematopoietic stem cell transplantation (HSCT) is a cellular therapy that involves the transfer of an immune system from a donor to a recipient through the replacement of these cells. It represents the best option of curative treatment for malignant and non-malignant hematological diseases. Materials and Methods: bibliographic search was done using the PubMed service of the National Institute of Health of USA and consulting the guidelines of the Department of Health of the Argentinian Republic. HSCT is associated with an increased risk of infections, some of which are preventable by vaccination. The guidelines recommend repeated doses of mainly inactivated vaccines to achieve long-lasting immune responses. Objective: the efficacy of immunization is often hampered by graftversus-host disease or severe opportunistic diseases. Conclusions: the recommendations of vaccines for recipients of allogeneic HSCT and the discussion of the challenges and future directions that lead to the immunization of these patients are of vital importance.
