Cardiotonic glycosides inhibit the sarcolemmal Na+/ K+-ATPase and cause an increase in intracellular Na+, which reduces Ca++ extrusion through the Na+/Ca++ exchanger. The result is an increase in sarcoplasmic reticulum (SR) Ca++ load and cardiac contractility.
